Eligibility for Home Support

How do I know if I qualify for these home support services?

To know if you are eligible to receive home support services, you must first go though the intake process with Continuing Care.

Continuing Care intake number: 1-800-225-7225

Should you get their voicemail simply leave your full name and phone number and a member of the Continuing Care intake team will call you back and ask you a few questions.

A Care Coordinator will then arrange a time to meet with you to complete an assessment. The assessment will determine what care services you are eligible to receive based on your needs.

Once a Care Coordinator determines you qualify for home support services, they will then prepare a service plan referral for our home support agency and email it to us.

Once our agency has received this referral, we can then start our process to input you into our system.  We will then contact you and let you know when your services will start.

Adult Day Program
